Membership and Pricing
There is an enrollment fee of $100.
The enrollment fee will be waived if you sign up by December 1, 2025.
Patients 18 - 44 years old
$75 monthly
Patients 45 - 64 years old
$100 monthly
Patients 65 years and older
$125 monthly
